Thyroid Profile Test (T3, T4, TSH): Normal Ranges, Hypo vs Hyperthyroid Chart

The Thyroid Profile evaluates the pituitary-thyroid hypothalamic endocrine axis. The pituitary gland secretes Thyroid Stimulating Hormone (TSH) to instruct the thyroid gland in the neck to synthesize and release Triiodothyronine (T3) and Thyroxine (T4). These hormones govern basal metabolic rate, cardiac rate, thermoregulation, and lipid catabolism. An inverse relationship exists: when the thyroid is underactive, TSH spikes; when overactive, TSH drops.

When and Why Doctors Prescribe Thyroid Profile (T3, T4, TSH)

  • Unexplained weight gain, persistent lethargy, constipation, dry skin, and hair loss (Hypothyroidism symptoms)
  • Unexplained weight loss, rapid heartbeat (palpitations), heat intolerance, tremors, and anxiety (Hyperthyroidism symptoms)
  • Menstrual irregularities, heavy periods, or fertility workup in women
  • Routine annual titration for patients taking Levothyroxine (Thyronorm / Eltroxin)

Test ParameterStandard UnitMale Normal RangeFemale Normal Range
TSH (Ultrasensitive Thyroid Stimulating Hormone)mIU/L0.45 – 4.500.45 – 4.50 (Pregnancy trimester targets vary: 0.1 – 2.5)
If High: Hypothyroidism (Underactive thyroid): Pituitary produces excessive TSH trying to stimulate a sluggish thyroid gland.
If Low: Hyperthyroidism (Overactive thyroid) or excessive Levothyroxine drug dosage.
Total T4 (Thyroxine)mcg/dL4.8 – 11.64.8 – 11.6
If High: Hyperthyroidism (Graves disease, toxic multinodular goiter).
If Low: Hypothyroidism, severe non-thyroidal systemic illness.
Total T3 (Triiodothyronine)ng/dL80 – 20080 – 200
If High: T3 Thyrotoxicosis, Hyperthyroidism.
If Low: Hypothyroidism, chronic starvation, or severe systemic inflammation.

How to Prepare for Your Thyroid Profile (T3, T4, TSH)

  • 1Give your blood sample in the morning (7 AM to 10 AM) as TSH exhibits diurnal circadian rhythm.
  • 2If you take daily thyroid medication (Thyronorm, Synthroid), take the blood test in the morning BEFORE swallowing your tablet, unless your doctor specifically advises otherwise.
  • 3Stop taking Biotin (Vitamin B7) hair/nail supplements 48 hours prior, as Biotin interferes with lab immunoassay analyzers and produces false TSH results.

Frequently Asked Questions (FAQs)

My TSH is 7.5 mIU/L but T3 and T4 are normal. What does this mean?

This is termed "Subclinical Hypothyroidism". Your thyroid gland is beginning to underperform, so your pituitary is working overtime (elevating TSH) to keep circulating T3/T4 in the normal range. Your doctor will assess whether treatment is needed based on symptoms and pregnancy status.

Why are TSH requirements stricter during pregnancy?

During the first trimester, the fetus depends entirely on maternal thyroid hormone for neurological brain development. Most endocrinologists and gynecologists aim for a TSH strictly below 2.5 mIU/L during early pregnancy.
